What does the Ebola virus cause, and how is it spread? How does it affect our eyes? What is the Ebola virus? The Ebola virus, also known as hemorrhagic fever, infects humans and has an average fatality rate of 50%. It first appeared in 1976 in the Democratic Republic of the Congo. Since then, 23 outbreaks of the virus have been recorded in Africa, with the most recent one occurring in West Africa in 2014. For more than a year since the World Health Organization declared the Ebola virus a public health emergency, new cases in high-risk countries, such as Liberia and Sierra Leone, appear to be declining. Image: Ebola virus strain The virus spreads from person to person through direct contact with bodily fluids such as blood. Laboratory studies have not documented airborne transmission of the virus. Typical symptoms of the virus include high fever, headaches, sore throat, muscle aches, vomiting, diarrhea, and rashes on the body. In advanced stages, reduced kidney and liver function is observed. These symptoms are also accompanied by bleeding. These symptoms usually appear about 8–10 days on average after infection with the virus. After recovery, certain side effects have been reported, such as joint pain, anorexia, psychological and mood disorders, and uveitis (inflammation of the uvea of the eye, which contains many blood vessels). This iris delivers nutrients to the retina and improves the contrast of the image we see by reducing the amount of reflected light entering our eyes. How does the Ebola virus infect our eyes, and what does it cause? The eyes serve as an entry point for the virus, just like the nose, the mouth, and open skin wounds. As mentioned above, bleeding is a characteristic feature of the virus. Bleeding can be internal (in the mucous membranes, skin hematomas), but it can also appear in the white part of the eye (sclera). Traces of the virus have also been found in tears, as well as in the aqueous humor, which flows in the anterior chamber of the eye. If the aqueous humor does not circulate properly, it causes an increase in intraocular pressure. According to studies, this complication is directly linked to uveitis. Research has shown that even several months after Ebola survivors have recovered, complications—such as visual disturbances and inflammation within the eye—can still be observed. Among these, it is worth noting that 40% of Ebola survivors experience dark spots in their visual field (scotomas), blurred vision, pain inside the eyeball, and inflammation, such as uveitis and scleritis (inflammation of the sclera). The researchers in this study report that these complications can occur even 9 months after recovery. What are the symptoms of Ebola affecting our eyes? Ocular symptoms of the virus may include common conjunctivitis, while in advanced stages, severe bleeding may occur in the conjunctiva (the mucous membrane covering the sclera). Extreme effects of the virus on the eyes also include choroidal hemorrhages, which can potentially even lead to blindness. An interesting study in monkeys showed that a few days after infection with the Ebola virus, edema (swelling) appeared in the eyelids. Traces of the virus can also be found around the optic nerve, causing changes primarily in patients’ peripheral vision. Other symptoms affecting the eyes include vision loss, photosensitivity, and excessive tearing. As we have seen, there are many symptoms and complications that the Ebola virus can cause in our eyes. What is striking is that the virus’s effects can persist for quite some time after the patient has recovered. We await further studies to determine the clinical characteristics of uveitis (as the most commonly reported complication), as this is a priority for ophthalmologists. Similar findings apply not only to survivors of the virus but also to all carriers. In any case of suspected viral infection, the patient should immediately receive appropriate treatment, and the competent health care authority must be notified so that the necessary measures can be taken to prevent further spread of the virus.

The vitreous body is located in the inner cavity of the eye, between the crystalline lens and the retina. It is a clear gel that gives the eyeball its shape. It consists mainly of water (98%), but also contains sugars and collagen. The composition of the vitreous remains constant, unlike other parts of the eye, with the result that any substances (e.g., blood or cells) that enter the vitreous are either absorbed very slowly and with difficulty, or they remain and are removed surgically (if necessary).
