Vision with \u201cflies\u201d\u00a0<\/p>\nThe term flies refers to the presence of dark spots in the vision, which can be very annoying. Their appearance is due to liquefaction of the vitreous body and their treatment is a controversial issue. Surgery to treat them is possible, but is rarely recommended by ophthalmic surgeons, as they do not consider it necessary and take into account all the potential risks.<\/p>\n
These risks refer to retinal tears or detachment, bleeding or postoperative endophthalmitis. Therefore, if there is a choice to treat the condition and in the absence of objective parameters indicating the need for surgery, many ophthalmologists are reluctant to perform surgery.<\/p>\n
However, there are also cases of patients where the size and location of these dark spots create a serious problem in vision. In these cases, finding a treatment, even surgical, is essential.<\/p>\n
Vision with \u201cflies\u201d\u00a0<\/p>\n
A large study conducted around this topic and published in the American Journal of Ophthalmology1<\/sup> showed that patients were willing to sacrifice one year of life, out of every ten years of their remaining life, in order to get rid of the symptoms associated with flies. Furthermore, the patients in the study were willing to risk a 11% chance of death and a 7% chance of blindness to get rid of the aforementioned symptoms. Of course, whether these results correspond to reality is a matter of debate, as a very important, unmeasurable factor influencing the desire for surgery is the patient's personality. Indeed, unlike conditions such as retinal detachment or a hole in the macula, in this particular condition surgical treatment is often the result of the patient's desire rather than the ophthalmologist's suggestion.<\/p>\n Often, when the appearance of these dark spots in the patient's visual field is not caused by pathological causes (e.g. uveitis), it does not affect visual acuity. The patient experiences some discomfort and feels that he has a problem with his vision but the discomfort is not due to a decrease in vision, as on examination he appears to see normally. However, research has found that light scattering\/diffusion is significantly higher in eyes with \u00abflies\u00bb.<\/p>\n The question, therefore, for many ophthalmologists remains \u00abIs the flies an eye condition or just an annoyance\u00bb?;<\/p>\n The usual treatment for flies simply involves information and reassurance but there are other treatments such as YAG laser, cataract surgery with anterior vitrectomy and posterior vitrectomy. Ophthalmic surgeons who choose to use one of the above methods to treat this condition usually prefer posterior vitrectomy. This procedure, which is also used to treat other conditions, is a routine procedure with a low level of risk and high success rates, particularly with modern minimally invasive sutureless vitrectomy and the use of very thin 25 gauge instruments. Consequently, patient satisfaction is high2<\/sup>.<\/p>\n In contrast, the use of the YAG laser in this case has limited application. It has not been widely accepted, as its results are limited to a small number of patients to whom it has been applied. The most comprehensive relevant study to date, which has been conducted using questionnaires in postoperative patients, has shown that the use of the laser was in no 100% case successful in eliminating symptoms. Relative improvement was observed in 1\/3 of cases but a 7.7% observed worsening of the condition3<\/sup>.<\/p>\n References:<\/p>\n 1.
