Retinal detachment is an emergency ophthalmological condition that requires immediate surgical treatment. The retina is a membrane on the inside of the eye. It covers the entire inner surface of the back of the eye. Its name is derived from the ancient Greek word \u00abamphi\u03b2\u03bb\u03b7\u03c3\u03c4\u03c1\u03bfn\u00bb, which means \u00abnet\u00bb. This is what the ancient Greeks called this tunic, because this was the image it gave them necrotomically. Then the Latins used the corresponding Latin word 'retina', which means exactly the same. So this net, the retina, collects all the images that the person wants to see and sends them to the brain, to the visual cortex at the back of the brain, in the occipital lobe.<\/p>\n
Urgent problem<\/strong><\/p>\n The retina, in order to function properly, must always be in its \u00abstuck\u00bb position. If for some reason it moves out of position, i.e. becomes \u00abdetached\u00bb, then it ceases to have a connection with the central brain and thus cannot send images and light. In addition, many of its cells begin to atrophy and die.<\/p>\n The body in its attempt to repair the problem begins a healing process called productive retinopathy. During this process, membranes are created that are designed to repair the problem. But unfortunately, instead of helping the condition, they make it worse and, instead of reattaching the retina, they pull and tug it out of place, with the end result being complete and total detachment.<\/p>\n These two events, the necrosis of the cells and the formation of membranes, make the treatment of retinal detachment extremely urgent. In this case, the earlier the diagnosis is made and followed by quick treatment, the better the results will be.<\/p>\n Macular stain<\/strong><\/p>\n Here we should also mention the issue of detachment or not of the macula. The macula is the centre of the retina. That is, it is the central area, it is a normal part of the eye which is responsible for the sharpness of vision. The macula is what makes us see details, letters, numbers and features of the person we are talking to. Therefore, as long as the retinal detachment remains at the periphery of the fundus of the eye and does not approach the macula, the eye is not at risk of losing vision after a successful surgical repair.<\/p>\n However, when the macula is detached, the patient will experience a sudden loss of central vision and will need urgent surgery. Persistence of the macula away from its location means necrosis of cells that will lead to a reduction in central vision, i.e. visual acuity after an otherwise successful surgical treatment.<\/p>\n In simple words<\/strong><\/p>\n The causes of retinal detachment are many. The main one, however, is the fracture etiology. That is, this detachment that follows a retinal tear, which occurs after a pull of the vitreous body on the retina that \u00abtears\u00bb it, creates a tear, a fissure, and that is why it is called fissure-induced.<\/p>\n But let us explain in more detail the whole route of its challenge. The eye is a \u00abball\u00bb filled with a transparent \u00abjelly-like\u00bb substance. This \u00abjelly\u00bb is called the vitreous body or simply vitreous.<\/p>\n When we are young this jelly is hard and firm, like all the other tissues of our body (skin, fat, muscle). With age and as the tissues of the body loosen, so does the vitreous and eventually becomes fluid, \u00abwatery\u00bb.<\/p>\n The initial symptom of this liquefaction is \u00abflies\u00bb! These... flies are condensed areas of the vitreous that have not yet liquefied and are swimming in the already liquefied areas. The next consequence of liquefaction is a reduction in the volume of the vitreous. Due to liquefaction it loses some volume, shrinks and as a result it begins to separate from the inner wall of the eye, i.e. the retina.<\/p>\n Most of the time this happens without symptoms and without consequences. In a few cases there are... flashes, lights or lightning, which usually accompany the flies. Why do we have flashes and lightning? The retina is a nerve. Only it's not a nerve that carries pain, it's a nerve that carries light, so when it's stimulated, instead of pain, it produces light signals.<\/p>\n Thus the pulling of the vitreous onto the retina can be translated as pulling the skin with a pair of tweezers. And just as the tweezers will create pain, pulling the vitreous will create light! And while most of the time we eventually get relaxation of the pull and relief from symptoms, in a few cases (about one in 1,000 cases) the pull is so strong that it will create a crack, which if not treated promptly with a laser will lead to retinal detachment.<\/p>\n Sugar<\/strong><\/p>\n Other causes of retinal detachment are sugar (diabetes mellitus), which due to the problems it causes in the eye, leads to retinal detachment. Also trauma to the eye, either from a \u00abclosed\u00bb injury (i.e. when there is no open wound but only crushing and squeezing of the eyeball) or from an open injury (when there is an open wound), causes detachment. Previous eye surgery increases the likelihood of detachment and, finally, myopia is a predisposing factor for retinal detachment, albeit a minor one.<\/p>\n In the operating room<\/strong><\/p>\n The treatment of retinal detachment can be done simply with laser or surgical intervention may be required. Laser treatment is applied to very incipient fissures where the lips have not begun to elevate or, if they have, this elevation is quite limited. Laser application does not require an operating room, it can be performed in a well-equipped clinic, but it does require limited activity on the part of the patient in the days immediately following. Therefore, if the detachment is not diagnosed early and progresses, then surgical treatment is required. Two techniques are used in surgical treatment: exovolumbar and endovolumbar.<\/p>\n \u25cf In the first one, the surgeon tries to press the outside of the eye wall and get close to the detached retina. He or she uses different sized silicone \u00abcushions\u00bb called scleral grafts. These implants are fixed with sutures onto the white wall of the eye called the sclera, which is why they are called scleral implants. They are then covered by the transparent membrane of the eye, the conjunctiva, again with sutures.<\/p>\n Inside the eye and after the retina is attached, laser photocoagulation or cryotherapy is applied in an attempt to create a scar around the original crack and thus eliminate the cause of the detachment. This is the technique that has been practiced for over 60 years. From its inception to now, very many technological and technical improvements have contributed to increasing its success rates from 50% initially to 85-95% nowadays.<\/p>\n \u25cf With the intravitreal technique, also called vitrectomy, the surgeon moves the ectopic detached retina back into place from inside the eye. Using three entry portals in the white wall of the eye (sclera), he inserts instruments that allow him to perform manipulations from inside the eye. Then, and once the retina is in place, laser or cryotherapy is also applied here around the crack responsible for the detachment to create its tightness and stability.<\/p>\n Finally, a special gas that is absorbed in twice or four times as long as air will fill the eye cavity to keep the retina stable for 15 days until it is permanently stuck. This air has the same role as the plaster on the broken bone that keeps it immobile until it sticks. And just as a limb (arm or leg) with a cast cannot move, an eye with gas cannot see clearly until the bubble is absorbed by the body.<\/p>\n This endovascular therapy is a newer treatment and in recent years it has been used more and more extensively. It has equally good success rates, reaching 95% with a single operation and also has the advantage in recent years that no sutures are used. Its disadvantage over the older technique is that in patients over 40 to 45 years of age it will initiate the process of their cataract maturing much more quickly than if they had not had surgery.<\/p>\n Postoperative complications and new surgery<\/strong><\/p>\n In both techniques there is a period of 3-5 months, during which the patient -despite the initially successful operation- is at risk of retinal detachment, so the patient should again be treated promptly and immediately surgically. In these cases, treatment is always carried out from the inside and in the most difficult cases, instead of gas, silicone oil is used, which - unlike gas, which is absorbed in 15 to 30 days - remains and offers stability for as long as the surgeon deems necessary. The disadvantage of silicone is that a new surgical procedure is required to remove it.<\/p>\n In conclusion, it should be stressed that retinal detachment has long since ceased to be a disease with a poor prognosis leading to blindness. Early diagnosis first and foremost and prompt and correct treatment by a qualified ophthalmologist at the appropriate eye center will offer anatomical success rates of up to 95% with the first operation and almost 99% after reoperation in case of complications.
