Glaucoma is one of the leading causes of irreversible vision loss worldwide. It is a chronic disease of the optic nerve, associated with increased intraocular pressure and progressive destruction of the optic fibres. If not treated in time, it can lead to severe vision loss or even blindness. Modern ophthalmology has turned in recent years to innovative, minimally invasive glaucoma surgery techniques<\/strong>, known internationally as MIGS (Minimally Invasive Glaucoma Surgery)<\/strong>.<\/p>\n

What are the MIGS techniques<\/p>\n

MIGS are a new class of procedures that aim to reduce intraocular pressure in a safer and less traumatic way than traditional surgical methods, such as trabeculectomy. These techniques are characterised by smaller incisions, reduced recovery time and a lower risk of complications. Their goal is to improve the normal drainage of aqueous humor from the eye without creating large external filters or scars.<\/p>\n

Advantages of MIGS<\/p>\n

Newer glaucoma surgery techniques offer significant benefits for patients.<\/p>\n

  1. Safety: due to their micro-invasive nature, the risk of contamination, bleeding and hypotonia is reduced.<\/li>\n
  2. Rapid recovery: Patients return to their daily activities much faster.<\/li>\n
  3. Reduced need for medication: after the surgery, there is a significant reduction in the use of anti-glaucoma colloids.<\/li>\n
  4. Combination with cataract surgery: many MIGS techniques can be performed simultaneously with phacoemulsification, saving time and reducing the overall risk.<\/li>\n
  5. Normal drainage: MIGS utilize the eye's natural drainage pathways, preserving the natural physiology of the eye.<\/li>\n<\/ol>\n

    Indications for the application of MIGS<\/p>\n

    MIGS are mainly indicated for patients with mild to moderate open-angle glaucoma, where medication is not sufficient or causes side effects. They are also ideal for people undergoing cataract surgery and suffering from glaucoma at the same time, as both problems can be treated at the same time.<\/p>\n

    Main MIGS techniques<\/strong><\/p>\n

    There are various techniques and devices that fall into the category of MIGS, each with a different mechanism of action and scope of application.<\/p>\n

    1. Trabecular Bypass Stents (e.g. iStent, Hydrus Microstent)<\/p>\n

    These devices are placed at the level of the ethmoidal network and allow the free flow of aqueous fluid to Schlemm's system. They have been shown to be effective in reducing intraocular pressure, especially when applied in parallel with cataract surgery. The iStent is one of the most well-known implants, while the HydrusMicrostent offers expanded passage of fluid through the Schlemm's canal, increasing drainage.<\/p>\n

    2. Canaloplasty<\/p>\n

    Canaloplasty is a minimally invasive technique that restores the normal function of the Schlemm's canal. A special microcatheter is used to open and stretch the canal, improving the flow of aqueous humor. Its advantage is that it does not require a permanent implant and offers stable pressure reduction with a low complication rate.<\/p>\n

    \u00a0<\/strong>3. XenGelStent<\/p>\n

    The XenGelStent is a soft, biocompatible tube that is placed to create a new outflow channel for fluid from the anterior chamber to the suprasplenic space. It offers gradual pressure reduction with mild postoperative inflammation and a very good safety profile. It is particularly useful in patients with moderate to advanced glaucoma who do not respond well to medication.<\/p>\n
